临床候选药AZD 3759的发现与评价

 

  根据近年来的统计报告表明,肺癌患者日益增多,尤其表皮生长因子受体突变激活同时伴有脑转移和软脑膜转移的患者。此类患者预后不良,而且对于该适应证,尚无药物获批。对于此类患者而言,现有靶向药物(第一代:吉非替尼、厄洛替尼,第二代:达克替尼、阿法替尼,第三代:奥希替尼)效果欠佳,即使剂量超过规定标准。


  2015年10月22日,美国化学会官方期刊《药物化学杂志》正式发表阿斯利康中国创新中心、阿斯利康瑞典研发总部、瑞典卡罗林学院、瑞典卡罗林大学医院的论文,报道了一种在研新药(AZD3759)的开发和评价。


  为了克服中枢神经系统转移,需要中枢神经系统和外周都有足够活性的药物。作者对上百个化合物进行对比筛选,最终发现化合物AZD3759。经过临床前动物模型证实AZD3759在脑脊液、脑组织中可以获得与血浆相近的药物浓度;随后,通过小鼠移植中枢神经系统转移模型进一步证实AZD3759可以有效抑制脑组织和脑膜转移瘤的生长。


  AZD3759的化学名为:4-[(3-氯-2-氟苯)氨基]-7-甲氧喹唑啉-6-yl(2R)-2,4-二甲基哌嗪-1-羧酸酯(简称:1m,代号:AZD3759),与吉非替尼等小分子靶向药物一样,属于表皮生长因子受体酪氨酸激酶抑制剂,作者通过对吉非替尼的化学结构进行一系列优化改造(1→1a→1b→1c→……→1m,参见图1),综合优化分子大小、极性、刚性等结构参数,使碱性、水溶性、透膜性、稳定性等生物物理化学各个性质调整至有利于穿过血脑屏障的最佳状态,口服后可以有效穿透血脑屏障,全面优化设计成功地使AZD3759在血液和中枢神经系统不同环境有效浓度达接近,0~7小时脑组织、脑脊液与血液的游离浓度之比均达1.3(厄洛替尼、吉非替尼、阿法替尼0~24小时脑组织、脑脊液与血液的游离浓度之比分别仅为0.11和0.29、0.021和0.088、0.0066和0.27),并且能够诱导小鼠模型脑转移灶明显消退(参见图2、3、4)。

Recent reports suggest that an increasing number of patients with lung cancer, especially those with activating mutations of the epidermal growth factor receptor (EGFR), also present with brain metastases and leptomeningeal metastases. These patients have poor prognosis as there are no approved drugs for these indications. Available agents have poor efficacy for these patients even at well above their standard dose. Herein, we report the discovery of (4-[(3-chloro-2-fluorophenyl)amino]-7-methoxyquinazolin-6-yl(2R)-2,4-dimethylpiperazine-1-carb oxylate 1m (AZD3759), an investigational drug currently in Phase 1 clinical trial, which has excellent central nervous system penetration and which induces profound regression of brain metastases in a mouse model.
